Shriners Children’s Hawaii and its Honolulu medical outreach team are returning to Guam Oct. 26-30 to provide free medical consultations for youth, according to the Department of Public Health and Social Services.
The Honolulu medical outreach team will be providing free medical consultations for children up to 17 years old with orthopedic conditions, healed burns, or sports injuries at the Northern Region Community Health Center in Dededo.
The Shriner's team was scheduled to assist in Guam in July, but the clinic was canceled due to Typhoon Bavi. Public Health is bringing the Honolulu team back to assist with the clinical backlog from the disaster.
For new patients, applications may be picked up at the Northern Region Community Health Center in Dededo, Room 30, Monday through Friday, 8:30 a.m. to 11 a.m. and from 1:30 p.m. to 4 p.m.
Parents and guardians may also request to receive the application by emailing DPHSS’s Medical Social Services staff, Cherisse Santiago at cherisse.santiago@dphss.guam.gov or Nicole Loren-Duplito at nicole.lored@dphss.guam.gov
Applicants must submit the completed application along with their child’s birth certificate, a parent identification card, and proof of legal guardianship, if applicable. A physician referral is not required, and Public Health says self-referrals are welcome.
